VIU Tuition Waiver Program Authorization for Release of Information

I hereby authorize the collection and disclosure of my personal information between the Ministry for Children and Family Development (MCFD), the Ministry of Social Development and Poverty Reduction (if applicable), VIU Student Residences (if applicable), and Vancouver Island University (VIU) as it relates to the University’s VIU Tuition Waiver Program and to records such as:

• Confirmation of my eligibility for the VIU Tuition Waiver program as a current or former youth in care as defined by the Child, Family and Community Service Act (CFCSA)
• Confirmation of my eligibility for the VIU Tuition Waiver program as defined by the Ministry of Social Development and Poverty Reduction Child in Home of Relative program (CIHR) for at least 12 months (consecutive or accumulative in any combination).
• Educational information submitted to the University as part of the VIU Tuition Waiver intake process
• Notification if I am a recipient of the VIU Tuition Waiver program from the University
• Ongoing communication between Vancouver Island University staff and recipient of the VIU Tuition Waiver program throughout their studies at VIU.

I consent to this information being collected and shared to determine eligibility for the Vancouver Island University Tuition Waiver program and for consideration of potential financial support to be determined through the University. I understand that this personal information will be used solely for the above purpose.
