This feedback form will be used for the purposes specified and not for any other purposes. Completing this form is optional. You are free to stop filling in the form at any time. Your responses are confidential. Authorized personnel from the Office of the Associate Vice-President, Student Affairs, the Student Affairs Foundation Plan Oversight Team (SAFPOT), and delegate(s) assigned by the Associate Vice-President, Student Affairs will have access to the raw data, and your unedited written comments. We will anonymize your comments in case they are shared with a wider audience, by removing references to specific people, programs, or other information that appears as if it may identify you. To protect your confidentiality, your responses will be combined with other responses, for reporting, in groupings of 10 or more. Any personal information is being collected under the authority of Section 26(e) of the Freedom of Information and Protection of Privacy Act (FOIPPA).
